Back to School Checklist
From the RPS website.
When parents register their children for school, don’t forget:
- Proof of current address
- Withdrawal papers and transcript from former school
- Last report card
- Birth certificate (original copy)
- Social Security card (original copy)
- Immunization record (current)
- School Entrance Health Form (MCH 213 F Revised 4/07)
- Current Individual Education Plan (IEP) for Exceptional Education students
- Current 504 plan (if applicable)
Remember, the state requires that all rising sixth-graders receive the Tdap immunization prior to the opening of the school.

Additional Information:
Regarding School Supply Lists, make sure to get the one for your specific school as they are different than the main list!!! I bought several unnecessary items when I used the general list. And it turns out each specific teacher may make additional requests.
It’s also very helpful if you include a box of quart- or gallon-sized ziplock bags to your pile. I probably went through 200 of them in my 5th grade class last year.
They actually request those now. very handy!
